The type of personal information I collect
No personal information is collected on my website. I currently collect and process the following information prior to and during therapy:
Personal identifiers, contacts and characteristics (for example, name and contact details).
Summaries of therapy sessions.
Clinical questionnaires.
How I get the personal information and why I have it
The personal information I process is provided directly by you for one of the following reasons:
Contact information in order to send and receive information for example regarding appointments and the service.
Summaries of therapy sessions as a professional requirement.
Sharing of Personal Information
No information will be disclosed to a third party without your permission, except under very rare circumstances such as notes being requested by the courts, or if I have serious concerns that you present a danger to yourself or to others. In such cases I reserve the right to inform the appropriate agencies (usually your GP or other healthcare professional) to ensure the safety of all concerned. Information may be shared with a clinical supervisor in accordance with regulatory requirements on an anonymous basis.
Under the General Data Protection Regulation (GDPR), the lawful basis I rely on for processing this information is contractual obligation.
How I store your personal information
Personal data held about you (forms, questionnaires or therapeutic notes) will be stored securely in locked cabinets or on a cloud-based notes secure record system. Records will be kept for 7 years after therapy has ended before being securely disposed of.
Your data protection rights
Under data protection law, you have rights including:
Your right of access - You have the right to ask for copies of your personal information.
Your right to rectification - You have the right to ask me to rectify personal information you think is inaccurate. You also have the right to ask me to complete information you think is incomplete.
Your right to erasure - You have the right to ask me to erase your personal information in certain circumstances.
Your right to restriction of processing - You have the right to ask me to restrict the processing of your personal information in certain circumstances.
Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.
Your right to data portability - You have the right to ask that I transfer the personal information you gave me to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, I have one month to respond to you.
